Read MoreEvaluates, plans, directs and administers individualized therapeutic treatment programs aimed at the restoration or improvement of function, or the prevention of disability. Writes evaluations, progress notes/reports, discharge summaries and maintains patient files. Counsels patients and/or family members regarding Rehabilitation diagnosis, plan of care and prognosis. Maintains current knowledge of regulatory/legislative policies as they apply to the profession of Physical Therapy and adheres to same.
Qualifications
Graduate of Accredited Physical Therapy Program, Current NJ State License in Physical Therapy plus a minimum of one year previous work related experience. BLS certification within first 3 months of hire. Must hold specialty certification approved by APTA in area of practice or as deemed appropriate by the leadership staff (e.g.: CHT- Certified Hand Therapist, OCS-Orthopedic Certified Specialist, PCS-Pediatric Certified Specialist, NDT-Neuro Developmental Technique)
For EIP employees only: a valid driver's license with no pending violations and an acceptable driving record as stipulated by SJH policy and, if applicable, the insurance carrier, for those required to drive a hospital vehicle.
