Read MoreThe Ambulatory Patient Navigator I supports coordinated, patient-centered care across the ambulatory Primary Care network through patient outreach, navigation, care coordination, and transitional care support activities. The role supports Advanced Primary Care Management (APCM), Transitional Care Management (TCM), preventive care initiatives, chronic disease management, referral coordination, and care gap closure workflows. It is designed to improve patient outcomes, continuity of care, quality performance, and patient experience. The Ambulatory Patient Navigator I collaborates with providers, nursing, operations, and interdisciplinary teams to support value-based care initiatives and Patient-Centered Medical Home (PCMH) objectives.
Qualifications
Education
- High School Diploma or equivalent required.
- Associate’s degree in Healthcare Administration, Public Health, or related healthcare field preferred.
Certifications
- Completion of Medical Assistant Program OR valid/active CMA Certification preferred.
Experience
- Minimum one (1) year of experience working in healthcare, ambulatory outpatient practices, patient access, care coordination, EMS or related field preferred.
- Experience working within an electronic medical record preferred.
Knowledge, Skills, and Abilities
- Strong communication and interpersonal skills.
- Strong organizational and customer service skills.
- Ability to manage multiple priorities in a fast-paced ambulatory environment.
- Ability to work collaboratively within interdisciplinary teams.
- Knowledge of ambulatory workflows, patient navigation, and value-based care initiatives preferred.
- Bilingual abilities preferred based on practice needs.

Pay transparency: St. Joseph’s Health provides a salary range to comply with New Jersey Law. The rate of pay for each position will be determined based on a variety of factors including the candidate's relevant experience, qualifications, skills, etc.” The salary range does not include incentives, differential pay or other forms of compensation.
Organization
St. Joseph’s University Medical Center is an academic tertiary care medical center and state designated trauma center, located on the Paterson campus, regularly accepts referrals of difficult or unusual cases from other hospitals and physicians and performs both complex and routine procedures.
The Medical Center offers a full complement of specialty and subspecialty services including ambulatory, behavioral health, cardiovascular, emergency/trauma, internal medicine, neuroscience, oncology, orthopedic/spine, rehabilitation, surgical specialty, women’s health, imaging and pediatrics.
