Nurse Navigator Referral Management

The Nurse Navigator is the clinical anchor embedded within the Central Referral Management team, working side-by-side with referral representatives to convert incoming referrals into scheduled, completed patient encounters across St. Joseph's Health service lines. While referral representatives manage the operational workflow of intake, data entry, and scheduling logistics, the Nurse Navigator applies clinical judgment to triage referrals by acuity and appropriateness, resolve clinical questions that stall conversion, and escalate urgent or complex cases for timely intervention. The role bridges referring providers, receiving specialty practices, and patients to remove clinical barriers to care, shortening the time between referral submission and patient access while safeguarding quality and safety.

Qualifications
Bachelor's degree in Nursing (BSN) required; three to five years of relevant clinical experience, ideally spanning ambulatory, case management, or care coordination settings. Current, active licensure as a Registered Nurse in the State of New Jersey. Basic Life Support (BLS) certification through the American Heart Association, required within one year of hire. Working knowledge of Microsoft Excel and Word, with proficient data entry skills. Experience with referral management, scheduling, or EHR platforms (e.g., Kyruus, Epic, or equivalent) strongly preferred. Strong clinical judgment and comfort making triage-level decisions independently and under time pressure. Excellent communication skills, with the ability to work cross-functionally with non-clinical referral staff, physicians, and office staff.

Pay transparency:  St. Joseph’s Health provides a salary range to comply with New Jersey Law.  The rate of pay for each position will be determined based on a variety of factors including the candidate's relevant experience, qualifications, skills, etc.” The salary range does not include incentives, differential pay or other forms of compensation.
