Job Overview
Responsible for identifying, establishing and maintaining relationships with referring hospitals, nursing homes, emergency medical service agencies and communities within the region. Develops, implements and evaluates trauma prevention programs for each major age group including geriatrics. Facilitates transition of care for the geriatric patient. Develops and monitors the geriatric call back program.
Designs, coordinates, implements and evaluates trauma injury prevention programs and community outreach programs for the pediatric, adolescent, adult and geriatric populations. Facilitates the transition of care of the geriatric patient. Ensures the call back of patients post discharge and facilitates problem resolution for the patient. May require the scheduling of patient appointments, re-calling the patient to the ED, etc. Completes geriatric screenings for ED patients. Participates in educational program for staff and other community forums sponsored by St. Joseph's Healthcare system. Identifies funding resources and participates in the completion of grant applications. Collaborates with local agencies and facilities to enhance prevention programs. Participates in the orientation of new ED volunteers. Participates as a member of the ED and Trauma quality improvement programs. Maintains an active role in Passaic County Safe Kids. Meets all required competencies for department, unit and/or hospital. Responsible for performing all other duties as assigned. Special Projects
Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's degree in Nursing or closely related field, Master's degree preferred, and three to five years of previous work related experience. Registered Nurse Licensure in the State of New Jersey is required. Certified Emergency Nurse certification preferred. Requires analytical skills with the ability to exercise sound judgment, attention to detail and problem resolution. Must possess strong customer service, communication, organizational & interpersonal skills.
