Job Overview

Evaluates, plans, directs and administers individualized therapeutic treatment programs aimed at the restoration or improvement of function, or the prevention of disability. Writes evaluations, progress notes/reports, discharge summaries and maintains patient files. Counsels patients and/or family members regarding Rehabilitation diagnosis, plan of care and prognosis. Maintains current knowledge of regulatory/legislative policies as they apply to the profession of Physical Therapy and adheres to same.

Responsibilities

Evaluates patients and provides Plan of Care in accordance with individual patient diagnosis. Provides therapeutic interventions for patients. Provides education and counseling to patients and/or families including: nature of disorder, prognosis, type of therapy to be performed, home programs, safe us of adaptive equipment. Completes paperwork according to departmental/regulatory standards, including: evaluation reports, progress notes, discharge summaries, charge entry, statistics. Assures financial viability of patient accounts based on payer requirements. Maintains current licensure requirements and updates clinical knowledge by attending continuing educational activities. Provides follow up in-services to staff. Meets individual productivity standards. Provides referrals to other agencies, services, departments within or outside the scope of St. Joseph's Healthcare System and is a resource for patients/families as needed in this regard. Participates in departmental meetings or reads minutes as necessary to assure compliance with disseminated information. Participates in staff and program development. Responsible for performing all other duties as assigned. Meets all required competencies for department, unit and/or hospital. Special Projects

Qualifications

Graduate of Accredited Physical Therapy Program, Current NJ State License in Physical Therapy. CPR certification within first 3 months of hire. FIM certification to be taken following employment and training at Wayne, then certification must be taken through the rehab program (Uniform Data System for Medical Rehab-UDSmr). Valid driver’s license required and comply with Qualifications to Drive a System Vehicle as well as all policies as outlined in the SJHS Fleet Management & Safety Program for EIP employees only.
